Right here and now whether you're in Iraq or California or Georgia or the jaws of a shark or the closet hiding from a human monster. Somewhere, someone is experiencing more horror than you care to believe. Just because you haven't been there, doesn't mean that it doesn't exist. I have been to MY version of Hell and it began when I was an innocent child. What kind of God allows that? The same God who delivers me from it. He didn't put me here. I chose.

Following planets have been described in Hindu scriptures:
Hell (Narak)
Earth (Mrityulok)
Seven heavens (Swarg)
Baikunth
Golok
The soul is the divine entity that travels to these planets encoutering life and death of the body. The next destination is decided by the deeds (karma) of the mind. The final destination, the goal is to get to Baikunth or Golok to serve God.
The location of the planets can be described by God realized saints or God only.

It's not a Where, it's a How. It's a state of being in which the consciousness/soul is separated totally and eternally from God. An "alternate state" if you will.

Hell is where the fire burns inside and outside the bodies, raw nerves exposed, screams and moans echo in the ears, the body feels each thing with great sensitivity........ Hmmmm, this doesn't seem like a bad place to me, sounds like a very blissful place, where do I sign up? ~ : )

Some of the answers so far are interesting but too concise to get to the concept. Various theologies have held out 'hell' as an after-life sojourn. Some treat as transient one till rebirth and others as an eternal abode of 'sinners' as punishment meted out by God on the Day of judgement.
As a concept, it can be thought of as a deterrent alternative to noble life as a human being. Rewards and punishments has always been part and parcel of organised society and the concept of a pleasurable or painful after-life, evolved to ensure orderly behavioural pattern in the society to maintain tranquillity.
Heaven or hell as a defined space with the attendant paraphernalia, has helped by and large the orderly behaviour of people for ages, since the same served as a rule for the masses with varying levels of intellectual elevation.
Viewing the concept from a pure psychological phenomenon, we would find that any wrong-doing carries with it a guilt complex, which torments the doer. Every negative behaviour, not only impacts the victim or the society but affects the person more. The torment could have been avoided by non-engagement in negativity or could be mitigated by making amends or when not possible engaging in positivism in future. Till then the torment continues and this state, I think, is hell.
