Currently, for a land passage, you need your permanent resident card (green card) and additional identification. IF you use your Cuban Passport by itself you will have to apply for a visa.

If you fly you need to use a passport.

your current PASSPORT and your proof of permanent alien US status should be all you need. You may want to check with the PROVINCIAL authorities in Canada where you will first enter Canada from the US if you need any immunization document or visas.
