I hate being thrown, there is a million different things that can happen. A lot of times its not only my weight, but the weight of the opponent and the energy generated by the opponent that hits you when you hit the ground. Botht hte Judo and Wrestling throws are my least favorite to be on the receiving end of.

I would have to say the Uchi-mata and ippon seoi nage. There's nothing more dangerous than being dropped on your head onto a concrete pavement by an inexperienced thrower. Most times their knees buckle under the weight from an incorrect stance and you both go down while you're still flipping in mid air, you'd be lucky to end up with just a broken collarbone or arm and not a broken neck or a fractured skull.

depends on the situation and by whom

the slam depending on what surface you are slamming them and what part of their body. for instance a seoi nage of concrete and the person lands on their head could be serious or fatal injuries, but lets say they land on their shoulders and it was on grass then it would be less serious

a straight right by some professional boxers can leave an everyday person easily ko'ed, broken jaw etc...max baer literally had a deadly punch

thai kicks are simliar to being hit with a baseball bat you can crack ribs, break arms if done with enough power and if you connect to the head its lightsout. but its often hard to connect a clean kick to the head

it just varies

slam
the throw and slam trump the punch and kick because instead of being hit with a fist or a leg your getting hit with a freakin planet, there is a lot more momentum and impact involved.
the slam trumps the throw because a slam involves the weight of both people, while a slam only involves one persons weight. also I have seen many people thrown and slammed on their heads they usually get hurt with throws, but not always, the only person I have ever seen not get knocked out from being slammed on their head is fedor, and fedor isn't exactly a normal human being
