In bulk, any gas supply company can provide you with certified "aviators breathing oxygen." If you're an FBO and have an account, they'll deliver it, which beats trying to fit a 200+# bottle of oxygen into a hatchback.
Smaller volumes, such as would typically be found on a light aircraft ranging from a Bonanza to a G-500 are sometimes sold by volume in weight, and sometimes you'll find a neat place that simply services the oxygen for a flat fee.
The key difference between medical oxygen and aviator's breathing oxygen is the moisture content of medical oxygen. Aviator's oxygen is moisture free, which in the event of a leak, as regularly happens, prevents valves in the system from corroding and thereby sticking.
I always felt a little guilty using it for the torch, but sometimes we have to use what's on hand.
No, you don't need any paperwork. Go AirGas, and you can buy or lease a bottle without displaying much more than the cash to pay for it, so long as you're not trying to stick it into the trunk of a taxi.

Aviation breathing oxygen is dry to keep the moisture from freezing your equipment. Medical oxygen is also dry, so dry in fact, moisture must be added when you are breathing it to keep from drying out your nasal passages. If you by OXYGEN from a welding supply it will be just the same. Be sure to specify oxygen and not just compressed air.
If you have the money you can buy it.

It`s no different from what grandma uses. Can be bought at your local pharmacy or local welding supplier
