If it's white, it's calculus, if it's black/green, it's tartar... and the only way to remove it is to have it professionally done by a dentist or hygienist.
After it's all gone and your teeth have been polished, schedule your next cleaning appointment before you leave the office so you'll stay on top of it... and it won't be as bad next time.

Some people just grow tartar faster than others. It's what the bacteria in your mouth build from the mineral content of your saliva. If you had fewer bacteria, it would help. The suggestion to get a Sonicare is a great idea to reduce the bacteria present before they can form the tartar. You mention six months between cleanings. Quite a few of my patients have to come in every 4 months or even every 3 months for a cleaning. No, their insurance doesn't cover it, but their insurance doesn't care if they lose their teeth. (In fact, they'd probably be happy if they did - no more claims to pay!) I have a small handful of patients who come in more often, although that is usually THEIR choice. I have a couple who come in once every month for a two minute cleaning of their problem areas - usually the lower front teeth. As long as they are regular patients on a 3 month routine, we give them that visit for free.

Yeah. Go to the dentist and get those teeth of yours cleaned professionally; possibly scaled. He/she will get all the plaque off completely if they're a good dentist. In the future, get rid of the plaque while it's still soft; or, better still, don't let it form at all.

A professional cleaning is the best thing.
But they do have picks in the dental section of most stores that you can use to remove hard plaque.
The best is to stay on top of it with daily flossing.
