It was a woman I saw driving the car so I'm assuming it was either her son or husband who received it, and since most are awarded posthumously it would possibly be a hand salute rendered to the family of the recipient. I don't know if that breaks any military protocol or ettiquette.

Since you're asking this question, I presume you're not a punitively discharged veteran. You therefore EARNED the right to salute, as it is the privilege of warriors.
I see no problem with you rendering a salute...and I'm sure any family member driving the vehicle has had it happen before. I'd probably bust one off as well.

It is not only proper but required for a military member to salute a holder of the Medal of Honor. The award is what is saluted and not a rank just as an officer's rank must be saluted but not the person. If you are a prior service member you are not required to salute anyone or anything-is it a proper showing of respect Yes but it is not required since you are no longer subject to military regulations.

A four star general is required to salute a private with the medal of honor. I am retired now and even though I don't wear the uniform anymore, I would come to attention and salute with pride. He or she is one of our nation's very best.

After the shock wore off from seeing it, yeah I would regardless if it was family or I was in uniform or not. Kinda like flag plates on a car, but with some extra umph.

As former military myself, I honestly do not know the technically correct protocol in such a situation. However, I probably would have acted as you did. MOH recipients are truly the finest of our finest, and are due our utmost respect in any and all possible situations.
