Suppose some has a 10 year visitors visa in which they can stay in the US 6 months at any given time. If they stay from Jan to June when is the next time they can return to the US for another 6 months.

It's up to the people at immigration to allow him into the country and the length of the stay.

It is a good idea to stay outside the US for as long as you were in.
The visitor must satisfy the officer at the border that the are not living in the US, and that they have a home outside the US they will return to.

What you are referring to used to be called a " multiple indefinite". They can return the following day because they have that particular visa which does not place restrictions on when they can re-enter country

I would recommend for my own experience to stay outside the US for 3 months. Of course, if the customs official ask you for how long are you going to stay say 2 weeks.

There isn't a specific time period requirement. They would have to go to the US embassy (sometimes appointments are needed in advance) and re-apply for a 6 month permission- basically they would just need to have sufficient proof that they will return to their country after 6 months.
