A somatoform condition is any condition in which psychological anxiety, depression, etc. manifests as physical symptoms, such as nausea, heart palpitations, shortness of breath, etc. Hypochondriasis and conversion disorder are two examples.

The phrase "somatic disease" has two definitions: In medicine, somatic diseases are all diseases that are not psychological; in psychology, somatic disease occurs when someone's psychological state of mind affects their health. If you are asking about the psychological meaning - it is usually applied to situations where a person gives up hope of overcoming a problem.
