In a fight you don't think, it all comes down to body reflex that's a result of yoru training.

The best and most effective way is by sparring, without sparring it's hard to know how your body will react in a fight.

If you mean gloves for fighting then yes you probably knocked the thing all over the place. That will change in time and with practice although it is harder to direct or hit the bag from certain angles with gloves on. As for offense and defense at the same time I am sure you have heard, "a good defense is a good offense" and there is a lot of truth to that. Just as some of the people have already said it comes with time, practice, and experience.

Some fighters have a harder time with their offense or defense so at my school we have them fight one sided. If a fighter is poor defensively then we have him fight defense only and he is only allowed to counter punch or counter kick. If he is poor offensively we have him fight with someone that is not be aggressive or offensive minded. By doing this you can more easily develop a fighter's offensive or defensive skills and abilities. Once they can do that then the rest comes more easily with training and experience whereby they can go back and forth from one to the other and make adjustments as the situation dictates.

If your arm = elbow hurts you are not keeping your punches to the center. Keeping punches to your center will help to keep elbows from hyper extending. Other advice is to spar. If you can't spar well you wont fight well. Caution sparing is not the equivalent of a fight in the street. That is a whole different animal. Lastly Train and Train and Train.

Defense=Offense
Offense=Defense

When you block, attack at the same time. When you attack, block at the same time and so on.
