The second sentence is correct. The way to check is to remove the "grandma and" temporarily; you would say "he would go down to London" but you wouldn't say "him would go down to London".

The second is correct.
If in doubt, break the sentence down into the people who would go down to London.
Grandma would go down to London
He would go down to London.
You wouldn't say "Him would go down to London"
it's "Grandma and he" or "He and Grandma"... whichever you prefer....but not "HIM".
