Deacon (greek - diakonos / διάκονος - from the Bible)
Priest (greek - presbyteros / πρεσβυτερος - from the Bible)
Bishop (greek - episkopos / επίσκοπος - from the Bible)

This is the hierarchy already described in "Acts" in the Bible.

Not all churches do this...but some do. At my dad's church, they practice a "shared leadership" concept...instead of a single pastor, they have a team of four that share the pastoral duties including teaching. They are accountable to each other for what they say and for how they practice what they preach.

At the church I attend now, the pastor has an "accountability team" that monitors what he says and how he acts. They went so far as to install accountability software on his computer...the accountability team can see all the sites he visits on the Internet.

I've attended churches that don't hold the pastor accountable...and I've known a few that don't practice what they preach. The usual reaction is that people who disagree with him do what you did...they leave the church. However, I think the better strategy is to confront him...singly and then as a group. A pastor is absolutely accountable to his congregation for what he says AND how he chooses to run his life!

that really depends on the Church. Small independent Church's are often not associated with a mother Church who oversees them. These smaller Churches are dependent on a board being formed with deacons and elders to oversee the leader and body. Churches lead by one man as pastor and leader without accountability to the body can easily control and manipulate the Church as claiming ownership. This is where you get many cult leaders from.

It depends on what religion. Some have them and some don't. Keep in mind that these things are created by man. The true Church is in Jesus. If the pastor doesn't do what he is supposed to be doing and anchoring his life in the bible, then ask him to step down. If you can't then take yourself to a church that you find follows what you believe.

The church usually has elders, or a board of directors who pay the pastors salary. If the church is part of a larger organization, such as Lutheran, Catholic, Episcopal, etc, there are rules of conduct and enforcement groups.

Every pastor can preach any damn thing he wants to. This is especially true in Baptist churches.

They can pervert the Bible all they want, so long as they are subtle and nobody in the congregation calls them on it.
