go to a place where you can paint ceramics/pottery. They have some in my city called Gotta Paint and The Mud Oven. Grandchild could paint a mug or X-mas tree ornament (if it's for the holidays)

Depends on the Grandma. My kid's great grandma is really into puzzles, and I know that you can have a picture of the kiddos put onto a puzzle.
If grandma likes coffee, put the picture of the kids on a coffee mug...
Have the kids do some artwork for grandma and have it framed with some current pictures of the kids...
A theme of something with Photographs in it is popping up here...:-)

Nicely framed photographs of her grandchildren or a mini album of their pictures with labels noting their names and ages and any activities they are doing in the pictures.
Pictures/photographs are a good idea in general, but if they have a stack of them they get shoved away and lost. Putting the pictures in a nice frame is nice to have on a table or mantel. An album noting the kids' details is especially nice since sometimes when you're showing pictures, grandparents need a little help in remembering details. Be sure to go with large print that is easy to read.

A lot depends on grandma's age. A nice lightweight white sweater will be worn year round when she feels a little chilly. A little gift basket that contains some of her favorite snacks and even a bottle of wine if she likes that will be enjoyed. A framed picture of you or even better one of both of you together. A gift certificate to one of her favorite places to shop or a restaurant where she likes to eat (or take her out to eat).

An art project that was made from her grandkids.
One year I got together pictures of our family and ironed them on to a pillow case. She loved it!
I also wrote a poem about her and glued the poem onto an ornament.

If you have money, flowers, bubble bath, picture in frame. If not make a frame for a picture and a card or get help and make a cake.

More than likely you may be just exactly that. Give her hugs and kisses show her you love her. Buy her a small bible sign it and give it to her. Set and talk to her us Ol Folks love to talk.
And when ya talk listen to what she says. Oh gosh i almost forgot take a pen and paper ask her what she remembers
about when she was young. And ask Family questions about her family and your Grandads family. names dates of birth death and who they married and how many children they had,
start a family tree. And keep a copy for yourself.

Get a multi-picture frame one with lots of places for photo's.And then with the help of other family members or if you have them....Copy some of your photos and assemble a memories frame.......I think that all grandparents sit and remember when.....And most love photos of their loved ones..Pictures of the two of you would also personalize it.....Have Fun.....................
