A 'state space' is simply all of the possible states that a control system may be in. Usually it's referring to a digital control system, but I've seen it used for large analog systems as well. The transition matrices you ask about are used to describe the manner (and under what conditions) a system will transition from one state to another. It's a fairly deep and math-intensive subject.
