It depends. For sure the child needs a passport, and possibly a visa depending on your destination.

In the US, in addition to any other requirements (such as passport and visa), if one parent is taking a child out of the country, you'll need written permission from the other parent or documentation that you have sole legal custody of the child.

I traveled out of the country once as a youth with my aunt & uncle, and they had some kind of form my parents signed off on giving them guardianship for the month we were away. I don't know if this was a requirement, or if my parents did it in case of medical or legal emergencies (even if not required, can be helpful in an emergency).
A child needs a passport, too.

You'll need a passport. To get a passport for a child under 14 you need BOTH parents present to file the application. If one parent cannot be there you need a notarized affidavit from that parent or an order by a judge.
If one parent has total custody of the child then you'll need a copy of the custody agreement.

The parental or guardian consent from which can be obtain thru a government website or an agency.
It must include (according to international laws):
-Child’s full legal name
-Child’s birthday
-Passport number or other legal document number
-Child’s birthplace and citizenship condition
-Your full legal name
-Your address in the country of residence
-Your passport number or other legal document number
-Your birthplace/birthday
-Your birthplace and citizenship condition
-Your relationship to the child
-Destination country, the number of days in that country and the place (address) that you and the child will be residing during the period of visit
-Your emergency contact information in that country or a legal affidavit of the exact location of your visits (if there are no contact information a lawyer should provide this document to the travel agents as well as to international authority.)
-Full legal name of the child’s parental or guardian
-Passport number or other legal document number of parent or guardian
-His/her relationship to the child
-His/her birthplace and citizenship condition/birthplace/birthday
-Finally, the signature of the child’s parent or guardian

depends--are you divorced from her father/mother, and is she under 18? if so, there are very intense requirements, so check with the airline. i think one thing is a note from the father/mother stating that you have his/her permission to bring you child out of the country. must be signed by them AND a witness (preferably a lawyer.)
