Many times it means the soldier was in a combat zone but died from natural causes, stupidity, accidents, etc. He wasn't shot, injured, or killed from direct fire or anything related to combat wounds.
Non-combat related deaths happen all the time even in the States - training accidents, heart attacks, etc.

I'm asuming you mean when you hear something about a soldier or marine dying overseas of non combat related injuries....
To put it the simply it's usually accidents. Like rolling a humvee, getting hit by a vehicle on base, a negligent discharge from your or another persons weapon, or to put it even more blunt, the way our higher ups explained it, "dumb sh*t".
%99 of non-combat related deaths are preventable and happen usually because someone wasn't paying attention or got compacent with their duties.
