Here's the thing everytime I see at school I can't help saying to myself "wow she's cute" and so I 'd like to thank you for brightening up my day whenever you pass by. Oh by the way I'm Mark... blah blah." Well that's it. Please give some comments on what I should say or should not say. And should I go for the number?

OMG you've got some work to do! if a guy came up to me and did that i would leave after he said " the reason why i approached you" dude not to be mean but thats creepy/lame.. you just need to be you and not try so hard... and don't go for the # wait till you see that she shows intrest and don't sound so smart yea girls like smart dudes but don't use such "right " language act like you know what your talking about and sound connifdent not so scared just actttt..... Not too bad. Maybe a little " heavy " for your first approach, but some girls like that. Oh, and you might wanna replace the word, " disturb " with the word, " interrupt ". Otherwise, she might think you're disturbed, which you probably are. And, I wouldn't go for the number unless you atleast get a smile or some other positive reaction...

Dont ever say "can i disturb you?"
If you want your magic act to be your ice breaker that's OK.
What you should do is this:
"Hey dont move for a sec", (magically pull a flower from her hair)" even without that flower you are still beautiful " " I'm Mark and you are?" "Thanks, see you around" then act like you are leaving and pause, "By the way may i know your email add?" (bring out a pen and paper and let her write it. ) (while she is writing say)"kindly include your mobile no. aswell"
Thats it. dont bring up any more cheezy lines. just leave. Dont worry she will remember you.

Try making a flower appear and then give it to her.
Also the "Umm the real reason why I approached you is that i would like to say something to you." is unnessicary, just start talking.

Lose the magic, its lame. Talk to her, "accidently" bump into her and say sorry, Ive seen you around blah blah blah and start a conversation.

you can drop the magic trick. all you have to do is say, excuse me, can i talk to you for a minute? she'll say yeah, then you'll say, i think that you are so pretty. and i was wondering if i could call you sometime.
