At the end of the year when you file your taxes ( or get someone, like H and R Block to file them like I did, there will be a place that asks if you had a phone bill an x amount of months. When you answer yes, the credit will go into your tax refund.

You listed your question under United Kingdom, but the phone tax was a US issue so I assume you are really in the US.

The phone tax refund was a one-time refund, and was available for 2006 returns (the ones filed early 2007) only. If you are late and haven't filed your 2006 return yet, you would include the applicable amount on whatever version of 1040 you file - there's a line for it in the payments section. If you had a phone but didn't have to file a return for 2006 because you didn't have enough income to be required to, you can still claim the phone refund using a 1040-T form. If you filed a 2006 return but didn't claim the phone credit and you qualify for it, you would have to amend your 2006 return using a form 1040X to claim it.
