Guilt, remorse, self pity, intolerance, impatience, hate, greed, self centeredness, sloth and procastination are possessed by all humans to some degree or another. You give yourself to God when you admit to these defects instead of trying to be perfect. God knows you will never be perfect. Instead of beating yourself up over your mistakes, indentify those defects you possess that cause you to backslide then ask God to remove them. It is a lifetime process. This holds true for all humans. Just know that if you do this, God will help you to become the person that He intended you to be.

Unfortunately, there is this idea that you can somehow "give yourself to God," and after that everything is fine and there's nothing else you need to do. That's not how life (and Time) works.
You don't really need to do anything special, just live your life by this simple precept: "Do only that which is right."
You can tell when you're doing something wrong; and when you have that knowledge that it's wrong, don't do it... Not because of God, but because of who *you* are. If you fail periodically, admit your error and try to fix it. This is called integrity.
It's not about what you have done wrong in the past, but what you're doing NOW (and on into the future).
If you follow this, you will know that your life has been given to God. The events and experiences that come your way, and the way you respond to them will make you a vessel through which "His Will" can get done. Besides, assuming you believe in God, what is there that isn't already His?

your relationship with God is a daily process, not a one time thing.
It is something you work at your whole life. First we must believe that God came to earth as man (Christ) and died for all our faults (sins) he paid the price of death that we deserved.
Because of this we ask for forgiveness in Jesus name.
Then we believe that he rose from the dead in 3 days. (Easter Sunday) or what is called the ressurection, this proves that he is the son of God. We believe this by faith.
If you believe in your heart, that Jesus came and died on the cross for your sins and that he rose again, and you say it from your mouth that he is Lord, than you will be saved from God's final judgement. (Romans 10:9,10) Thats how you give yourself to him. (you tell him in prayer)
Find a good place of worship where you can fellowship with other people of faith and learn more about what the Bible teaches.
Life is full of temptations and trouble, but we have Jesus' promise that He will never leave us nor forsake us. Pray for strength and wisdom to get through each day, and you will be more than a conqueror!

Keep trying and never give up. God will not give up on you. I went through the same thing. I accepted the Lord 5 years ago and 3-4 of those years i was back and forth. I felt filthy and horrible and i always thought, thats it for me, he will never take me back. I would go back saying this is it, no more, then the next day i was back to my old ways. Its hard, but you have to keep trying. Whats your other option, hell? Commit yourself everyday to Christ and if you fall again, get up, repent and try again.
Stay away, run away from situations that will make you fall

Get involved in a good Bible-based church. Seek out a group of your peers. Find someone in that group that is well grounded in the word. It should be someone that makes you think and feel, Wow, that's what I wanna be like.

U have to accept Him as ur Savior. Follow His path thru ur life. Confess ur sins, and vow to never do them again. Keep on doing the right thing. Then when ur time has come, u'll surely be in Heaven. ^_^

Fall on your knees and tell him that. Whatever after that happens in your life even the most painful and hurtful things,offer it up to him for your sins, and believe sincerely that he will take care of it. Just remember in his time not yours. Trust him completely. And of course to the best of your ability follow his ten commandments and pray. Repent, he understands everything about us, after all he created us. And remember he does not expect us to be perfect. If you happen to fall short, try, try again.
