1) Service Academy (West Point, Naval Academy - includes marines, Air Force Academy, Coast Guard Academy, Merchant Marine Academy).
2) ROTC (during college)
3) OCS/OTS (if you already have a degree)
4) Direct Commission (doctors/lawyers/nurses/etc, also medical mgmt, and a few related specialty fields; VERY seledomly is it used outside of that but it was a bit last year for Army reserve officers. This also includes battlefield commissions, but that isn't ever done anymore).
