I would get out and have my old friend drive the lady to a hospital.
If he saved my life once surely he would return the car.

They're at a bus stop! They don't need rides, they're planning on taking the bus. I would stop and say hello to all, but not offer a ride to any (they don't need it!)
Edit: That seemed a little mean, wow. Lol. Okay, so can I answer twice? I like both of my answers. I swear, I didn't peek at anyone elses.
I would give the keys to my old friend (I trust him to bring it back) and have him bring the old lady to the hospital. I would then sit and wait for the bus with dream guy.

I would drive the old lady to the hospital for the following reason.
1) My friend who saved my live would feel he/she had done a good thing by saving the woman's life as he had mine and he would want the old woman's life saved ahead of his need to get a ride.
2) If the third person was my dream woman she would know I had to save the old lady and would respect me for and and realize I am the right guy for her.
3) There is nothing more precious than human life and everyone regardless of age - is worth it rather that benefit the selfish interests to get a ride.
4) Who among myself, my love, and my friend would feel good about ourselves if we had a chance to save this old woman's life and we choose not to for the simple sake of another person being inconvienced.

After some careful thought my smart-*** answer would be to hand my friend the keys to the car and tell him to give the old woman a ride to where she needed to go.
-- I will call him for my car back latter--
And then say by the bus stop chating away with this dream person.

Ok, I would get out of the car, say hi to my friend and introduce myself to my dream man. Then, I would tell them that I only have room for one passenger in my car and I would just have to take the old lady to the hospital. I would then ask my friend to wait for the bus with my dream man. I would then thank my friend, say bye to the my dream man (maybe ask for his number???) and take the old lady to the hospital.

if we're talking about *my* car, first of all, I'd have already lost my dream mate when she saw me driving it, the old woman would have died WELL before I could ever get the stupid car to start again, and my old friend would say, " Jeez ! I saved your life and you're still driving THIS old wreck ?!?"
All three would just rather wait for the bus . . .
*You'd have to actually see my car to appreciate this answer* :-)

I shall lend my car to my dear old friend, so he can drive the old woman to the nearest hospital. Meanwhile, I will accompany my dream woman to wait a bus. This romantic situasion remind me of a classic love song, Rhythm in the Rain.

of course, you get out... have your old friend drive the dying person to the hospital, and then you stay and talk it over with the one you're dying to meet.
But if they're a star or something, they can call for a helicopter to rush the patient to the hospital, and that takes care of everyone.

i'll call 911 for that old woman, and see if she needs 1st aid/ warmth and wait for the medics to arrive, with that time i can have conversation with the person i wanted to meet, and if there is no bus, i'll do my friend a favor by taking her to her destination.
edit: i see a lot of people wished they'd be with the dream person. they're not sure yet if that dream person wants their company.
i wouldn't want to bother with another inconvenience a friend i am indebted with.
