There can be a number of reasons:
1. If it was only just completed, then some residual infection at the root tip may not have resolved, and a short course of antibiotics could help;
2. If it was completed some time ago, then perhaps a canal was missed. This need not be carelessness or negligence by the dentist, as sometimes the canal shapes can be strange and Xrays only give a 2-dimensional view of a 3-D object.
3. Another possibility is root fracture. It is usual after an RCT to put crowns on teeth to wrap around the weakened structures. If there isn't a crown, then a crack might develop down the side of the tooth, allowing bacteria under the gum and causing some swelling and pain in the gum.
I suggest you go back to the dentist and have these possibilities checked. If he still can't find the source, then ask for a referral to an endodontist (specialist in RCTs).

Yes! My dentist gives me a concoction of 2 Extra Strength Tylenol with 1 Advil - all at the same time - and it really works to kill the pain. It's comparable to a Vicodin without the side effects.

Yes, they may have missed a root. See the dentist that did the root canal.

Yes it could be an accesory canal (smaller extra one). see the dentist who did the treatment. But finish your current prescription of antibiotics if you have one and see if that clears it up.
