It's a usual procedure for your primary doctor to send a consultation form to another specialist. It includes your brief medical history relevant to the problem; your diagnosis, and reasons for the referrals are specified. However, this consultant can get other related information from the patient himself upon a physical exam. He might also obtain the info from the lab report which is in the computer.

Or he might require the whole chart. In the latter situation; sometimes, the doctor spends more time reading the chart than actual time with the patient.

That's why this is called a multidisciplinary teamwork of medical care.

They are allowed to keep what is called "work product" when they send out records. However very little if anything in a medical file falls under the definition of work product, so all information in your file can be sent to another doctor when you sign a medical release

No, that is illegal. Medical records have to contain everything that you've ever been diagnosed with or treated for.

It may be illegal but that doesn't mean it doesn't occur.
