I also wanted some sort of sleeve for my gown. We were having an outdoor wedding/reception and I thought it may get a little chilly later in the evening!

This is actually the dress I got, I ordered the one shown in lavender- but I got it in ivory. It was really very comfy and I got a lot of compliments, with all the beading it just shimmered like diamonds! If I were you I'd just Google "wedding gowns"... then you can look at different designers and websites... Designers like Mary's Bridal, Alfred Angelo, etc.
I would suggest NOT using David's Bridal. Yes, their gowns are nice looking and usually good priced, but I've heard nothing but bad things when dealing with the store (you can only get their items from an actual David's Bridal store)... I've heard plenty of horror stories, so I wouldnt think its worth using them.
You can also go to www.altavista.com, click on images, and search "Bridal gown with sleeves" and several pictures will come up... then you can browse through the pictures. When you click on a picture you like, it takes you to the website that the picture came from... you can alter your search too by searching "wedding gowns" "bridal gowns with cuff sleeves" etc.

My cousin tried on a dress in store, it was $3000, she then found the same dress being made by someone on eBay. Including shipping from China it cost her $189 (We're Australian), when it arrived she admitted the detail and quality was no where near that of in store, and some beading needed to be replaced, but her mothers a dressmaker and could easily fix it. Another girl I know bought one online, the back didn't do up correctly so she took it to a seamstress who replaced the zip with a beautiful tie up ribbon-back. Lovely. You can buy any dress that is sleeveless and then hire a seemstress to add the arms . This way you are not limited to very ugly gowns . That is what a very good girlfriend of mine did to cover up her tatoo's . Talk to a seamstress it is very resonable . Try Davids Bridal they do have a very large selection .

www. maggiesottero.com is the best.
I have a tattoo on the nape of my neck that I am going to have covered that day. They make a makeup that goes over it and does cover it up and they make it in several shades to match your skin tone exactly if you were interested in doing something like that. You could also try just getting one of the cute little sleeve attachments that bridal shops sell seperately, they just clip onto the dress. Davids bridal had some cute ones.

Try Alfred Angelo.com they have some very stylish and elegant dresses. Different then the normal gowns.

I am in the exact same situation as you!
What I've found is that all the freaking sites have the same sleeveless dresses and note "you can add sleeves".
What a pain!
I've found that going to a store has worked the best for looking at dresses WITH sleeves.
