I just learned this by attending a Jewish funeral.

When someone visits the site, he/she places a pebble to mark the fact he/she was there. In Jewish custom, it is said the deceased is forgotten after 12 months. The stone reminds the dead, he/she was not forgotten.

It serves two purposes--it gives comfort to both the living and the dead, the stone being a physical reminder of the living's presence and the deceased's memory.
