There are different answers because there are different types of photos. For example, I try to make every photo as clear as possible, but some people like to make the softer "art" photos. A photo can be beautiful, or interesting, or educational, or mysterious but it doesn't have to be all of the above. Personally, I just use them as a travel record.

Whatever you're shooting should be easy to identify. Not a lot of background distractions. Many times in photography less is more.
Find your subject, focus on it and let the background be a bit fuzzy so it will really stand out.
Make sure it's balanced and that the person looking at the photo will know what it is they're supposed to be looking at.

depends on what the photo is for.
if it's for some class you're taking, well then, in-focus, low grain, correct exposure, interesting subject, etc.
otherwise, it doesn't matter, whatever pleases your eyes. that's what's great about art. it's all up to you.
