It depends on what's wrong with it. Is the truck broken? Replace it. Is the bearing removable? Take it out and put in a new one. Are the ball bearings missing from the bearing itself? You probably have to replace the bearing, then. The raceway is probably worn enough that new ones would not fit well.

Take a good look at your board, and write down the manufacturer, the model name and number, and any other identifying information. Take it and the board to a skate shop or a sporting goods store with a good skateboard dept in your area, and go look for parts. If you have a good shop, tell the owner that you'd like to learn to take care of your own board, and you'd like to just buy the parts you need if possible.

Remove the nut, slide the wheel off , pop the bearing out of the wheel, and replace it with a new bearing.

If your bearings are just dirty (gritty) you can pop the dust seal out of one side of the bearing, wash the dirty grease out with degreaser, then re-grease it. Reinstall the bearing with the side that no longer has a seal toward the inside of the wheel so that it's not open to the elements.

That's what I used to do.

go to your local skateboard shop & buy new berrings. unfortunuately, at most places you can't buy just one, you have to buy a pack of 4. they aren't too expensive, & the staff usually puts it back on for you (:

Bearings are very hard to fix and aren't worth your trouble, just head down to you local skateshop and buy a single bearing shouldn't cost you more than 3 dollars.

You will have to replace it.
If it is inside the wheel, it will probably not be replaceable - whic is a drag. Some trucks have bearings that can be replaced, but you will have to check out a board store near you.
Good luck!!
