If you are not asking about the disabled person parking placard, then I've seen these decals on e-Bay.

The application for which they were use was to warn pedestrians that there might be a wheel chair ramp lowering beside or behind the vehicle.

Where I live (Alberta) you require a form signed by your doctor and a decal provided by the registration authority.

got to you local department of transportation (where u take ur driver test) and you can get a hadicap tags..........(must have letter from your doctor).I think that's what u mean???
