EFP's are being built in Iran and shipped to Iraq by the Quds Force of the Iranian Revolutionary Guards in support of Special Groups terrorists (could also be called Hezbollah) in Iraq.
The proof has been documented by the US Military. Iran is training, funding and supplying Special Groups. Also documented.

An EFP is a very low technoligy explosive devise. The main difference between it and an IED is that an IED just explodes trying to kill/damage people and equipment with frag and the blast. An EFP will primarly kill/damage people and equipment with a metal slug, often copper, that melts at the point of detonation and then cools off prior to the slug penetraiting the armor making the slug into a needle the rips through the armor.
An EFP can be made in any machine shop as long as you can get your hands on the explosives. Where Iran comes into play is that a lot of the artillery rounds that are covereted into EFPs come from Iran.
While most EFPs are made within Iraq, those locations close to the Iranian boarder will often attempt to smuggle them accross the boarder. This way their factory's wont be targeted by Coalition Forces.
When EFPs first appeared in Iraq, often Iranian 'civillians' would be cought with skematics or at these EFP factories training Iraqi insurgents on how to build them.

The main reason Iran is being suspected in the EFP issue is because they have found that most of the parts in an FP come from manufactures and plants in Iran. Also, mostly Shi'a have EFPs making it a connection to the Shi'a lead government in Iran.

The technology itself is fairly old and simple, the EFPs used in Iraq often made from simple everyday parts - metal pipes, tubes, copper lids, etc.
Iran is being suspected because they've been known to clandestinely aid particular insurgent and terrorist groups in Iraq before.
With most IEDs being rather crude and clumsy devices, who's to say someone didn't teach them this handy new weapon? Iran's Quds Force may not be the most likely group to have supplied or at least taught them how to build them, but they're certainly a sure name on any list of suspects.

Not sure. But, I've been told that some of the ones that were captured before use have markings in Farsi on them. Since everyone in the other nations surrounding Iraq (and the Iraqis themselves) speak Arabic, that's an indicator that the Persians (Iranians) made these things. Of course they could have also come from parts of western Afghanistan, but they would have to be transported from the Dari region of Afghanistan via Iran to get into Iraq.
