Q. "my mother lost my w-2, and they never sent me another. What should I do?"
A. "If you misplaced your W-2, contact your employer. Your employer can replace the lost form with a “reissued statement.” Be aware that your employer is allowed to charge you a fee for providing you with a new W-2.

....

You still must file your tax return on time even if you do not receive your Form W-2. If you do not receive the missing information in time to file, you may file you may use Form 4852, Substitute for Form W-2, Wage and Tax Statement. Attach Form 4852 to the return, estimating income and withholding taxes as accurately as possible. There may be a delay in any refund due while the information is verified. If you receive a Form W-2 or W-2C (corrected form) after you have filed your return using Form 4852, and the information differs from what you reported on your return, you must amend your return by filing a Form 1040X, Amended U.S. Individual Income Tax Return."

Q. "What happens if you dont do you taxes by october 15?"
A. If the IRS thinks that you were entitled to a refund, nothing happens. They do not send the refund, but they also do not otherwise punish you. However, if they think you owe them money, then they bill you for it, plus interest, plus penalties. If you file late and you owe them money, then you still have to pay interest and/or penalties. If you owe them money and do not pay at all, then they garnish your pay check or seize your assets.

If you filed for an extension but don't file your return by Oct 15, then it's considered late. If you don't owe anything, there won't be any penalty - if you didn't have enough withheld and you owe, there will be penalties but since you probably wouldn't owe much, they won't be huge, but will add on every month, so file as soon as you can in any case. Call your employer or former employer and request a duplicate W-2, or you can probably get a copy from the IRS.

File as soon as you can with an explanation why you didn't get it in by October 15. You can call IRS at 800-829-1040 and request W-2 information free.
