Photoshop is a very expensive photoediting program. If you need to ask what it is, you do not need it. It has many, many operations that the average amateur never uses. You cannot get it for free or very cheaply. The photo you give the link to has been converted to greyscale, then inverted to give a negative effect and the wording added. Many programs can do this - you could try Photoshop Elements 5 which costs about 20% of the full Photoshop - trial editions are available from the Adobe website.

Photoshop by Adobe is the most advanced and most successful digital still image processing program. Its lesser version is called the Photoshop Ellements. These programs are also compatible for Mac and actually works better on them. Yes - you can create photos that look like your sample and much more with Photoshop.
You can find the older versions for lower cost of about USD 200+ on Amazon.com - new ones are more expensive.
Thankfully, Adobe offers a free download for trial at the following link. Since it is an advanced program, I will recommend some tutorials at the link given below. Also look at a sample of what the Photoshop can do by artist Bert Monroy - each of these images were 'created' on photoshop (as against shot on camera and modified)

In order to create something like that, you definitely need a photo editor like Photoshop. That being said, like others have said, you don't learn something like that overnight. It's a VERY powerful tool, it's rather expensive, and takes a great deal of time to master.

Photoshop is considered the Cadillac of photo editors, it's what the Pros use.
You might want to start with Photoshop Elements 6. It can do a lot of what PS CS3 can do, and it's a lot cheaper and a little easier to use. You'll be able to create a photo like that with Photoshop Elements.
There is another program you should check out too, it's called Xara Xtreme....

It has similar features as PS but is a LOT easier to use, and it's very fast. PS requires a pretty fast computer with a large amount of RAM, otherwise it's pretty slow. Xara works differently under the covers, which makes it much faster.

You already seem to know what photoshop is. It is the software / program to create those images. Actually it is very powerful so it can be used to create just about any kind of effects.
But it is also very complicated. And all the names of the functions are confusing (like Gaussian blurr, unsharp mask, etc.)
You cannot get it for free. It is very expensive. You can get is somewhat cheaper if you are (a) student (b) teacher. If you know somebody who is a student (in college), they can often just buy it for you.
As for the how to, it is almost like learning math. If you take it step by step, it isn't all that difficult. But just like math you can't learn it in just few days. It take months of practice. The worst part is that if you don't use it over and over, you can easily forget how to use it.

since you gave a link to that picture.. photoshop is great. good for photo manipulations, colour editing etc etc. great program to play around with a picture
however, If you're talking only talking about photos, and not manipulations etc etc, there's this program for macs, Aperture if im not mistaken.
There's adobe photoshop CS3 extended, but it doesnt come cheap. =)
And dont be discouraged, start by exploring your photoshop (when you get it that is), play around with it, and when you're happy exploring all you can, there are PLENTY of tutorials on the net for you to learn from. just go to a search engine and type in "photoshop tutorials", and POOF.
