Just as anywhere else in the world, you will find the biggest and best medical facilities where the most people are ~ the major cities.
Australia has universal public health care for citizens and residents, which means you may not have to pay the doctor or hospital at the time of your visit.
This system is NOT available to tourists or visitors, so if you are a traveller make sure you have travel and health insurance.

You don't pay to use public hospitals anywhere in Australia if you are a resident of Australia. Queensland used to be special before Medicare as the lottery paid for public hospitals but now it is all covered by Medicare.

Not really sure what you mean, but all Australian residents have access to public hospitals and medical centres which are subsidised by the government - so minimal GP fees and hospital stay (if you are prepared to wait for a place). The major cities and most bigger rural centres have public hospitals or medical centres.

You don't pay to use the facilities in a public hospital in Queensland but I think you have to be a resident of Queensland
