Methadone is very effective and has a small side effect profile. But the best overall pain relief is Fentanyl which is available in a transdermal patch or a transmucosal swab.
Incidentally, CoCodamol is no more effective at relieving pain than paracetamol, but the side effects are significant. Therefore it should be taken off the market as there is no true therapeutic purpose for the medication.

there is no such thing as a favourite painkiller....it all depends on the type of pain, the circumstances in which it is occuring.
the group of painkillers called Non Steriodal antiinflammatory drugs (NSAIDS) is the most widely used worldwide and can take care of most simple aches and pains. These include- Paracetamol, a simple painkiller which should take care of most aches and pains, infact it is an excellent pain killer with a good safety profile, with very few side effects. infact we even use paracetamol for relief of pain from major operations. also in this group of NSAIDs- include Ibuprofen (Neurofen) and Diclofenac (volterol) which are also excellent pain killers.however they are not used widely in asthmatics- as can precipitate an attack, and also use in caution in patients with peptic ulcers, renal failure.
the other group of pain killers include Opiods and opiates (which include codeine, morphine, etc)
Codeine is also a good painkiller though widely used in combination with paracetamol (Co- Codomol). codeine like most opiods is best for dull aching pain rather than sharp pain. thus it is used in combination and is said to be excellent to relieve a tooth ache.
the opiods include Morphine which is called the king of all painkillers against which all painkillers are compared. it is usually reserved for severe pain- like after a major operation or in chronic pain of cancer.
tramadol is also used alone or in combination with paracetamol is good for moderate to severe pain though causes drowsiness.
well if it comes to me personally i would choose paracetamol due to its excellent profile- efficacy, safery and lacking side effects.
though since pain is subjective, each person percieves pain in a different way hence each person will have his or hers preference for a pain killer which will defer from person to person, so i wowuld say, there is no favourite pain killer!

Tramadol.
It stops most sharp pains, a lot more effective than any codeine based pain killer.
I also use Diclofenac, which is useful as a dull ache pain relief.
When all else fails, I use cannabis in large quantaties and that helps enormously, also takes my mind away from the situation

The best pain killer I've ever had was after my rear van door blew against my hip (the other car booters were worried about me & i'm not sure how i managed to drive myself home)
Had an X-ray because everyone was worried that i may have broke my pelvis, but thankfully it was ok, just alot of deep tissue bruising. (in laymans terms: hurts like a brake but isn't a brake)
The dr gave me co-dydomol (not sure on spelling) but it didn't touch the pain. I didn't get any sleep that night.
The following day a friend of mine gave me ONE of her pain killers. there was a thunder storm that night & i laid ASLEEP listening to it. (my ears must have been awake, lol) It was wonderful to get a decent nights sleep & be free from pain for a while.
The tablets name ....Kapake, = wonderfull stuff. :-)

Codeine, usually in the form of Solpadol, or the really strong co-codamols you need a script for. Or morphine.

Acetaminophen for particular...It extremely seems to help my issues...or perhaps even that Excedrin stuff because it style of feels to be a mixture of Acetaminophen, Aspirin, and Caffeine. That stuff knocks the soreness top out!

Nurofen Plus, although when I have a particulary bad back pain, I go with Ibuprofen.
