Health insurance never covers bills for services received prior to the date that the new coverage was obtained. The only exception is that COBRA is retroactive to the date of the qualifying event.

It depends on the plan. Most health insurance if it is thru your employer will cover a pre existing condition if she had continuous insurance. Call the company and find out.

No your current coverage is only from when it starts and may not cover her pregnancy at all depending on the policy and when she got pregnant compared to when the policy started.
