Right after approval of Form I-130, Petition for Alien Relative and Form I-486 Adjustment of Status, the spouse of US Citizen will receive Conditional Green Card. After 2 years of marriage, the US Citizen and the spouse file I-751Petition to Remove the conditions of residence. Both of you will go to interview and Immigration Officer will ask you some to tons of questions to verify that your marriage is not a fraud. Then the real green card will be issued based on the approval.
If anybody finds out that the alien entered the marriage to US Citizen to obtaining green card, they can report to the local immigration office. If IO finds out the fraud in marriage, they can revoke the green card and deport the alien.

About a year after getting married the immigrant gets a conditional green card in the mail. The conditional green card is good for two years. 90 days before the conditional green card expires the immigrant has to apply for a permanent green card.

So, the marriage has to last about 3 years before the immigrant can apply for a permanent green card. If her husband hasn't applied as of yet he's not permanent yet.

the alien (immigrant) is already able to file for 1-485 (adjustment of status) right after the marriage. the process takes 90-120 days. after that, the applicant will already receive the green card. =)
