Hi guys...
I am filling out a health insurance form to be waive from my school's insurance...I have a New York medicaid / Benfit card...and would like to know what is the insurance policy number and where it would be.. if it is located on the card?

my card has 19 numbers, where it says card number.....i am doing it online and in the box provided it only allows 18 numbers to be submitted.....

It's your ID number on the front of the card, about 8-9 digits long.

Your insurance id member number is the combination of letters and numbers - the AB12345C number. Should be towards the top left third of the face of the card.

All the numbers along the bottom, must be for state purposes, because with the exception of the SEQ number (the last two digits on the bottom right corner) we'd never had a need for them.

If you want to talk to a lawyer in Texas...then go get one. You won't find them here. Talk to social security office in your area about the card and your mother not giving it to you. For the birth certificate, you can order one from the county or hospital you were born in. For the medicaid card, you need to talk to the department of social and health services and social security. Tell them the situation so you can get another one. I don't really understand why a 17 year old would not know where they were born. Talk to other family members about it and see if anyone knows. Otherwise, talk to a social worker.

Try to put it in 18 number, sometime they don't count the last number

I looked at my medicaid card just to be sure and it is the 12 digits on the front of your card.
it looks like this...
