Unless he's flirting with you in some disgusting way...
yeah beer and viks don't cut it after a while with bad bad tooth aches. Go back and tell him you have a tooth ache...or maybe just go to a different dentist and tell him. It might be that you have to go to the emergency room if it's on a weekend (they have a dentist) someone i know had a very very VERY bad tooth ache and the vicodin was NOT working for him, he went to the er and found out he has two abses teeth.
Go to the dentist!! you don't understand how deadly this could actually be!
poor teeth condition leads to the poison in your teeth getting into your bloodstream and your heart! you could die!!! SERIOUSLY!!
...and start brushing your teeth and flossing more thoroughly at LEAST twice every day!!!

If he looked in my mouth, he'd die, be buried and turn over in his grave. I have at least $25,000 worth of work to do if I want to save my teeth. I take anti-depressants--give me dry mouth, so my mouth isn't being washed continually like everyone else's--that, and I drink soda.
Ok, the cute dentist with a foul mouth--Fire him. I do not visit any dentist who is not sympathetic to my pain. They say anything negative about the way my teeth look (do they think I'm blind? I know how my teeth look!) and that's their last appointment with me. There are better dental experiences than he's handing out, and I'd find one.
For me, going to a dentist is traumatic. If the dentist and his staff add to the trauma, they can just GTH.

I would be upset. Then, I would tell him off and find a new dentist.
With customer service skills like that, he deserves to go under.

Depends on his tone of voice.
I would consider it highly unprofessional for a dentist to say such a thing. Next time he says it, say whenever you get the chance that you don't appreciate his rude and insulting comments, and, if he continues anyways, say that if he doesn't like working on your teeth, you'll take your money elsewhere.
