It mostly depends on which airline you choose.
Often they can switch your ticket to the next flight, and there may be a fee depending on what kind of ticket you bought. Most really cheap tickets have a fee of $50- $75 if you make any changes, but the more expensive tickets usually don't have those fees. Cheap tickets are usually non-refundable but expensive ones can be refunded.
More than once, I've missed a flight with Delta or United, and they were very nice and put me on the next flight without charging any fees, even though the fine print on my ticket said that I was supposed to pay a fee.
On the other hand, I once missed a flight on Continental and they were total jerks about it. They made me buy a whole new ticket, and it cost twice what I had paid for the first one. I will never fly Continental again.

What you do is call the airline and get them to put you on the next flight and if you dont want the next flight then you can get a refund.

when you miss your flight... because you did not arrive the airport on time for your departure ( your fault) you will pay a no show fee and a rebooking fee. but when you miss your flight because your flight before that flight was delayed and you where check in as a check thru passenger they will put you to the next flight. and if the next flight will be the next day they should give you a free hotel accomodation and a free airport - hotel - airport shuttle. plus meals

Typically, you just go to the counter and let them know you missed it. They will arrange to get you on the next available flight. They usually do not refund you for this.

Depends on the airline and the ticket you purchased. Once all they did for me was put me on another plane the same night, another time I was charged a processing fee than I was allowed to get another flight.
