i don't understand hebrew in walla.com. can somebody help me with this, please.?

i have an account in walla.com. although i know my username, i can't remember my password. and when i click שם משתמש, seemed to be the 'forgot password button", i don't know what are being asked about the account information since it is written in hebrew. what does this mean? שם משתמש and תאריך לידה . please help. thanks

The first one means "user name".
Shem = name
mishtamesh= user
The second one means "date of birth".
Taarikh = date
laida = birth
