What is the best way to lighten dark photos?
Also, if a photo is a bit out of focus, what do you do to make it clearer/sharper?

Sorry (to some of the earlier contrubutors) ... but Brightness/Darkness, Auto Levels and some of the other suggestions you've already gotten are WRONG!

In order to Optimize your image (That's really what you're asking about), follow the steps in this tutorial.

It'll explain why you never use brightness/contrast, and auto anything ... until you've done the basics correctly.

It's a simple process - based on several years of teaching this stuff and dealing with my own images.

Blurry photos are usually a result of bad photography (poor focus, shutter speed too low, camera shake). You can make corrections, but they are seldom satisfactory. I throw out photos that are not sharp at 100% in Photoshop. For dodging and burning, I recommend reading John Shaw's e book "Photoshop Guide". which is available on line (www.johnshawphoto.com). This is written by a great nature photographer for nature photographers. John's books on Nature and Landscape Potography are outstanding. There is lots of good advise and many superb images.

In photoshop, go under Image then try either Brightness/Darkness...play with the sliders to see what you like. You can also try the Auto Levels under Image...if you don't like something, you can always undo....just save your original image...use "save as" when you get something you like...just in case you ever want to go back to original. You can go under Filter, Sharpen, Unsharp Mask...(think these are the titles) to make the image more sharp...try 125, 0, 3 for the three settings in that order...just learned these steps in my class! But if pic is too blurry, you may not be able to fix. Hope this helps.

If it's really dark then you can't use Brightness/Contrast to fix it. You'll need to go into levels and adjust it from there. If that isn't enough, then try curves.
For blurry, you might need to shrink the photo frame size a bit, like from 8X10 to 5X7 and then sharpen it from the filters.

One should always get it right in the camera.

Read and study your Owner's Manual and learn how to use your camera.

Then you won't need to waste time trying to make a mediocre photo into an average one.

You'll have more time for your family and your photography.

Lightening:

Image > Adjustments > Brightness/Contrast

Sharpen:

Filter > Sharpen. This can be repeated but too much looks odd.
