I remember being taught at school when I was like 8 or something that you place the cutlery down on the plate with the handles resting over the sides and the other ends in the centre. Then to show your appreciation for the meal you slot the end of the knife in between the prongs of the fork....... and the left most space meaning the meal was excellent, the middle one meaning it was ok, the right most one meaning it was unsatisfactory.
Oh and to show that you are resting but not done with your food you place the handles on the edge of the plate at the 4 & 8 o'clock positions with the ends pointing towards the centre.
It's weird that I remember this lesson taught once in my life ever and I can't remember where I put the remote 10 seconds ago, lol.

Once you have used the utensils, do not place them back on the table. While you are eating, if you want to continue to eat, place your knife across the top of your plate. When you are finished eating, place the fork and knife together on the plate, with the handles pointing to about 4:00. This is the American custom. To follow the English custom, do what angelcollides has told you above: when you have finished eating, place them at about 4:00 and 8:00.

the you are done place your knife and fork in a 12-6 position with the sharp bit facing towards 12. if your halfway through and need a break put the knife and fork 4-8 with the sharp bits inwards.
if you have many peaces of cutlery just remember to use them from the outside in.

Cross them over and put them on the plate if you're done & don't put them on the plate if you're still eating is what I do (I'm holding the fork usually).

Put the spoon and fork on your plate; put your napkin in your chair if you intend to return to eat more.

My grandmother was European born ans she instilled in me proper manners, but I don't recall this one, except that you don't set cutlery down on the tablecloth. When I finished I lay them across the plate. These days most wait staff ask before they touch your plate.

Well, I place them "gently" on my napkin. But only after I have used the napkin to dab the corners of my mouth, making sure to extend my pinky finger while doing so.
