A turn off is whatever turns YOU off. Everybody's different, some folks can overlook all the choices you put up, for others any or all are deal breakers. The first two choices you offer are pretty much hygiene related and can be modified... The other two are personality traits and really can't be changed too much.
Me? I have no shame, I will offer a mint in a heartbeat, tell a person don't get any stains on my couch, shut up and take a breather or drag them kicking and screaming into a conversation. Holla!
Honestly, aside from the hygiene thing(gotta have good hygiene) the things that turn me off are:
Being rude/inconsiderate
Being disrespectful
Being a know-it-all(especially when they're wrong)
Being a braggart
Other than that, I mostly ignore other people's foibles.

All the above and then some you have to feel the person out to see how she reacts to you dirty teeth would get to me but dont change your self to try and impress someone you will never be happy with that some people dont mind greasy hair bad breath talk to much or being to quiet if they truly are into you.

Someone who talks too much with bad breath. Oh, and it is worse if they talk about themself with bad breath.
