I consider it to be one that doesn't advertise, tourists don't know it exists, only locals go, and it's cheap and small. It looks like they're barely paying the rent on the place (in other words, no extra $$ to decorate).
My two favorites are Tres Hermanos, excellent Mexican food, and Small World, great gyros and falafel.

I went to a bar in N.C. that was actually named The Hole in the Wall. It lived up to it's name it was cheap and disgusting.
