A pap smear is very simple and takes less than 2 minutes to perform. An instrument called a speculum is inserted into your vagina and is used to spread your vagina so the doctor can see your cervix. After the visual inspection, the doctor inserts a long, very thin q-tip looking thing with a soft brush on the end. This soft brush is used to "scrape" some cells from your cervix, which are then sent to a lab to be tested. Then the speculum is removed and you're finished. Its a tad uncomfortable, but only while its being done. Once the doc's through, there's no pain, and its very important to have a pap smear done. Regular pap smears can help you stay on top of your reproductive health.

After you have the drape over your lower body and then legs in the stirrups the doctor will insert a special instrument called a speculum which looks like the bill of a duck. When they do the pap smear the doctor will take a special brush and insert it in to part of the opening of your cervix and take some cells. The cells then get put on a slide that then gets sent off to a lab for analysis. The results will come back in about 1-2 weeks.
You may notice a bit of bleeding after the test for a day or two and this is normal. If it goes further past that then let the doctor know what's going on.
It only takes about a minute to do the test.

I know it's scary and it's not cool how they get all up in your personal areas but it's really really necessary. I'm 24 and in the past 2 and a half years, I have not had a single test come back with good news. I'm in the navy so it's mandatory for me to get pap smears every six months until my results come back normal.
Ok, here's what's up... You should only have to get naked from the waist down. If they're doing a breast exam them full naked but with a gown. You sit on the edge of the table with your bum hanging over the edge and your feet up in the stirrups. It's going to be super uncomfortable. Just look at the ceiling and remember to breathe.
Then comes what's called a speculum. It's shaped almost like a duck bill and they come in different sizes. My doc always uses the smallest one since I've never had kids. They put some KY on it to make it go in easy and then they open it just slightly. It feels a little odd, like there's some pressure buildup. Then they're going to take this think that looks like a bottle brush only way way smaller and they scrape some cells for testing. This feels like really bad menstrual cramps. Then they're done. You should have your results back soon.
Now, don't be afraid. Cervical cancer is 100% treatable if it's caught in time and if you don't go... they may never catch it in case you may have it. I'm only 24 and my cells are abnormal. I've had to get some extra tests done and if I never went to the doc... I may have gotten full blown cancer.

I'm 21 and just had my first Pap. To be honest, I thought it would be a lot worse than it was. I put off having one for so long just because I was scared and anxious, but there is no reason to be. There are a range of speculum sizes, depending on you, and they are only up there for about a minute. I advise, just look away and pretend it's not happening. It's not painful at all, and I couldn't really feel any scraping. I just thought it was a little awkward, but then realised, hey the dr. does this all the time, so there is nothing to feel wierd about!

It may be uncomfortable and you may feel some pressure, but not painful as long as you have a good doctor. The speculum isn't that big. Some women can feel the scraping, but it doesn't feel like scraping, it feels more like tugging or touch from the inside. It doesn't last very long. Relax your muscles, think of somthing else, and it will be over before you now it.
bacis, but covers it all:

It is not painful but slightly uncomfortable. You dont need to worry about how big it is, as it is made for the job. It is the thought of it more than anything else that puts you off. It is a good idea to have one annually. I used to have it around my birthday, so I wouldnt forget. It is too important. Now I have had a hysterectomy, I dont need to have one any more, so I make my mammogram around then. It helps to remember. There is no need to be scared.

because of the fact it is not the least bit arousing, for one component. it is totally scientific. it is approximately as sexual as whilst a doctor pushes your tongue down with a tongue depressor with a view to look as your tonsils, or whilst the opthalmologist folds your eyelid returned to get a extra useful look at your eyeball. The gynecologist is only attempting to get a look at a physique area that may not almost as available as tonsils and eyeballs. so that they could use a gadget reported as a speculum, which seems extra like a duckbill than a penis. the only resemblance to sexual intercourse is this gadget is inserted into the vagina, and that's unquestionably the beginning up and the tip of all similarities. the sensation is plenty nearer to that tongue depressor i discussed than to intercourse. If intercourse have been in common terms as sturdy as a pelvic examination, the human race might have perished eons in the past, because of the fact no one might have bothered to do it.
