You have several options
http://www.lasvegasadvisor.com/transportation.cfm
this will explain them all.
You could take the city bus for $1.25 if you travel light(carry on luggage)
Both the #108 and #109 stop at McCarran. For info
http://www.rtcsnv.com
One bus or the other stops about every 6 minutes. If you're going to the South strip area, it's best to take the #109 Maryland Pkwy South bus out to the SSTT, this takes about 8 minutes, transfer to the Deuce(strip bus) which leaves about every 5 minutes during most of the day. This will cost you $3 total, $2.50 for a 24hr CAT pass, then an extra .50c for the Deuce, you will then be issued another 24hr pass that will cover both the Deuce & CAT. If you're staying on the North end of the strip, take the #108 Paradise, you can then transfer to the Deuce, when it stops at the strip(near the Stratosphere) -
For anyone going all the way downtown, both the #108 Paradise & #109 Maryland Pkwy "North" bus run from McCarran to Fremont Street & or the DTC >Get off at Fremont for most of the downtown hotels, stay on the bus for another minute and get off at the DTC if you're staying at Main Street Station or the California. This trip takes abour 45 minutes and costs $1.25 >Hint, take the bus, you will be checked into your hotel, while the guy waiting for a cab is still standing in line, for the privilege of paying $20-$25 to take a long cab ride to downtown.
>The lone exception to the Bus/Taxi/etc to downtown is the El Cortez, they have a free shuttle, for their guests to & from McCarran. The only problem with their shuttle is, that it only runs on the even hours(during the day) from the hotel to the airport and you have to let them know 1 day in advance that you plan to use the shuttle.
Taxi info - with a chart showing the price from McCarran to each hotel.
http://www.taxi.state.nv.us/faresfees.htm
It's a good idea to write down the TA phone number and carry it with you. If you feel the cabbie has cheated(long hauled) you, call the TA from the cab before you pay.
Unless you show up at McCarran between Midnight at 5am you're probably going to wait at least an hour in line for a cab, maybe two hours or longer on Friday/Satuday or when a big event or convention is in town.
IMO the airport to hotel shuttle is the worst way to go, "most" of the time. You wait for a shuttle that's going to your area, then you wait for the shuttle to fill up, then you wait while they drop everyone off. It's lovely.

That answer may depend on how much baggage you have.
The cab line snakes back and forth for about 4 rows. Each row is very long. All rows are always full. It does move fairly quickly. The total walk is almost a mile, which could be a problem if you have a wife who packs like she's going off on an african safari for 6 months. The cab will take you directly to your hotel.
You don't have to do anywhere near as much walking for the shuttle. You may have to wait a little longer - maybe not. The ride may take a little longer as the shuttle stops at other hotels. The shuttle will be the cheapest of the two.

As much as it sucks, you're probably better off taking a cab. It will run you about 20 bucks to most parts of the Strip. Mind you, on most days you will have to stand in a long line to get a cab. It sucks, I know, but I assure you it's no worse than any other of the options. I was in Vegas two weekends ago, and we paid 15 bucks from the airport to Paris. It blows, but until the monorail comes in, there isn't really any option.
The shuttles SUCK. I took one to get to New York New York - I paid 15 bucks, and the driver went to the OTHER end of the Strip first. It took like an hour and a half. I would have been MUCH better off just waiting for a cab. I know, I know - those lines are LONG. But they move pretty fast.

Shuttle is the best. If you ride a cab in Las Vegas, you are going to go broke. They are about 5 times more expensive than they were just 5 years ago. Why take an expensive and stinky cab from the airport for 26 bucks when you can rent a car for 26 bucks a day and go anywhere any time. Remember, Las Vegas strip is miles long, you'll not be able to walk it, trust me. I lived there.

I prefer the shuttle. It's about $11 I think and you can reserve a return shuttle as well. As soon as you walk out of the airport there will be some waiting for you with all the taxis and limos. The taxis have been known to drive the "long way" so they can charge you more. Could end up being more than the shuttle.

Either way there is good and bad. You will wait over and hour for a cab. The line keeps moving but there are so many people waiting. Shuttles are crowded and you can wait awhile too. You just got off a crowded plane ... not your best choice. Plus it seems no matter what hotel you are going to it will be the last stop for them. Go figure. No quick way unless you want to pay the extra for a limo. Might be worth it.

I use cabs from the airport to my hotel. Shuttles may be cheaper, but you stop at a number of hotels on the way to yours. Depending on which one you are staying at, it may take a while to get there.

shuttles stink. I've never taken a cab FROM the airport, we always do the walk-up flat rate stretch limousines outside of baggage claim that will take you to any hotel downtown or the strip for $42.

both are a nightmare at mccarran. Queues for taxis can exceed 1 hour because there is just one terminal and everyone comes out the same door.
Shuttles are annoying, most are not going where YOU want to go.
Book a limo before hand or take the bus would be my recommendation, otherwise you will have a lot of waiting to do.

If I'm by myself, shuttle. If with several people, a cab, since we share the fare.
