I think there are probably a lot. If not all.

Acting for many people is the way to express their inner fears, dreams and feelings.
Many shy people have decided to become actors just because of that freedom that acting can provide.
But since acting is so demanding - all you have on the stage is your body, your voice and you have to fight by using them as your only weapons- it seems that this battle cannot be given by everyone.
It needs courage, fire and strenght - physical and psychological - in order to overcome the fear of the unknown audience and start acting. That is the challenge of the stage.
I would love to see Robert de Niro on stage and Daniel Day Lewis, also. In fact they are fearful! But I don't know about any anxiety or stage fright that they may have.
On the other hand there other examples, such as Al Pacino who loves acting on stage because he believes his development as an actor has its potential on stage.
Finally, I believe that there are many factors that make an actor decide between screen or stage.
And we must always bear in mind that the screen gives the opportunity to a large number of people to watch an actor that in other circumstances they may never had the opportunity to do so.
And by making a film an actor can hope that his job will last more than a stage production.

Yes.
Donny Osmond. Saw him on TV talking about it.
Abraham Lincoln.
Bob Dylan.
Brad Pitt.
CHER.
ELVIS. ( I'm not making this up! These people really have/had S.Anx.Disorder and shyness issues!)
Harrison Ford
Jim Carrey (THis is one of the hardest to believe, but it is true. I saw him speak about it on 3 talkshows over the past few years. Acting is his way to become someone else who is NOT fearful, but in real life he is shy and was as a kid.)
Nicole Kidman
DeNiro
Gere
.... The list goes on but I googled and found some of this, others I remember from talk shows. Good luck. Use acting as your outlet to become someone else and talk about your fear. It will go away, promise.

There was a pretty famous one... known as Bob Hope.. He had to go back stage to vomit before his on stage appearance. So is not something uncommon for nervousness to take over at the wrong time.

I don't know about current actors, you usually find out this sort of thing in biographies and memoirs. Gary Cooper, a very famous actor in his time had this very problem, but he also had courage and overcame it.

There are famous actors who are guilty of all of the above, but have learned to overcome it.
I have a friend who stutters terribly.
His occupation? He is a professional singer, whose specialty is opera.
He has sung with the Metropolitan Opera, on Broadway, when get gets on stage , he deals with it and does a wonderful job.

I know Al Pacino doesn't do talk shows because of the live audience. He once did the Rosie O'Donnell show but it had to be pretaped in an empty studio.
