Do you think that one day at some point in time that all the chemicals that make you up could one day come together again and revive your consciousness, maybe in a cloud of chemicals like a gaseous cloud in space. Or maybe again as your self.

Well everything you experience know and imagine is merely chemicals in your mind just electricity traveling through some matter, if it all fell together again, maybe it would be you? because it's what You are.

I think that something like that is what happens when we pass out of the physical body.

no. that would be impossible, since the consciousness does not actually take up any space or time in the physical universe. therefore, once the body is dead, which i assume you mean, it is gone forever. it needs all organs to function, and they cannot come back. a body can be without a consciousness (it is even believed that consciousness, or awareness, if a fact of evolution only, and fairly recently, a matter of 5000 years or so). and consciousness can be without a body. however, the revival of death into life is impossible after a certain amount of time (in a matter of seconds to minutes), because once the cells die, they can never come back, which is why so many people with brain injuries either don't make it, or they make it, but have so much brain damage that they can't remember, be aware, or even function at all.

No. You're asking either the wrong question, or the right question wrongly. The part that is YOU is gone forever. Cloning or something could bring something that looks like you, but would in no way be you. If space were to invert, or there be an alternate universe, once again it would look like you, but not be you psychologically. Only you have gone through the experiences of your life.
