Absolutely possible, with support and lots of hard work on your part, you can and well achieve your goal.
Make sure the university you attend are aware of your difficulties, and hopefully they will set up some support systems while your studying. These supports can be either a tutor, note taker or class room support worker, access to computers and other adaptive technology.
You will need to have a very clear plan how you will manage your workload, and stick to a set routine, but always ensure you have time out for recreation, family and friends etc. A student diary is excellent for planning your workload.
Did the IQ test identify area's of strengths and weaknesses, e.g reading, maths or writing, the weaker area's is where you will need assistance, while your studying.
Please remember that IQ test are good for diagnosis and setting up access to support services etc, if the person tested is deemed to have a significant impairment, that requires them to have on going supports, other than that IQ test don't mean a whole lot.

I hope these IQ tests were done by a school psychologist and not some of these online tests that do not measure ability correctly. Intelligence is composed of many attributes and if you take one that measures just one attribute, it will not be an accurate measure.
I think you should base your hopes and dreams on what you can actually do, rather than what these IQ tests have told you. If you have had an IEP in school, every college has a disabilities office that will implement some of the accommodations on the IEP.
Also, most colleges and universities have some testing that will tell you the areas that you need to improve to make your college work successful.
I started in a community college for my first two years rather than a university because the classes were smaller and there were more remedial classes available. I went from a kid who was failing in high school to being very successful in college because of this. You might try this route.

using an IQ test to judge yourself mentally is not the way to go. IQ tests are unreliable; your IQ is able to go up or down very easily. personally, as a kid, i was labeled as below basic, and now, 6 years later, i am in AP classes in high school. proof that IQ tests do not work.
rather, just apply yourself to the material. what you have over many high IQ students is motivation, and that will get you to your phD. colleges do not use IQ tests to weigh admissions whatsoever, just grades, ECs, and most importantly, your admission essay. but if you apply and focus yourself, youll be fine.

Just go for it. Study hard, and no matter what your intelligence quotient, you will eventually get a PhD if you work enough. By the way, online IQ tests are garbage.
