Mountains form due to collisional forces between tectonic plates. The Appalachians were formed by Gondwanna crashing into Laurentia. The Himalayas are being formed by the Indian plate running into the Asian plate.

Mountains disappear because of erosional forces becoming greater than formation forces. Right now formation forces are greater than erosional ones in the Himalayas so that's why they are still growing.
