Your coolant is leaking into your piston.

This is usually caused by blown (damaged) head gasket.

Leaking head gasket, cracked head or something akin to that which is letting coolent leak into the cylinder and when you start it there is white smoke as coolant burns off. If you are lucky it will just be a head gasket.

start by checking you oil....if the oil isn't foamy or creamy, then you don't have to worry about the head gasket...whew.
if it only gives a little "puff" when you start it it is probably the guides for the valves....not unusual for any motor with high millage....
if the engine isn't using much oil and you aren't adding oil on a regular basis....i wouldn't sweat it...
