I've spent 28 years working at universities, so let me explain something that your counselor should have. If you want to be a plastic surgeon, you need to eventually get into a good medical school. In order to do that, you need to go to a very good college and do extremely well there.
I'm not saying that you can't do that, but so far, your GPA is barely passing high school, and you have a fairly low ACT score. Unless you go to a community college, you will be limited to a very few universities, none of which are very well respected. Getting into medical school from one of these would be extremely difficult.
So your choice is basically to decide between going to a four-year college or becoming a doctor. If you really want to become a doctor, community college, transferring to a four-year school in two years with good grades, would be your best bet.
Still, I looked on the College Board's website (www.collegeboard.com) and I put in your statistics. In the entire country, there were only 10 colleges (open admissions) who fit the criteria. All have open admissions, which means that they will accept anyone who graduated from high school. They were:
